Working in the form from your desktop

You can complete this form offline. Print these instructions to work with next to your computer.

To save the form:

  1. Select the form contents only (do not 'Select All' as this will save other webpage features)
  2. Copy and paste into a Word document.

To type details into the form boxes:

  1. Go to View in the Word toolbar, then Toolbars, and select Web Tools
  2. Design Mode iconIn the Web Tools box, make sure Design Mode is turned on (it should be highlighted)
  3. Then in the form itself, select the text area you want to type text into. It should now have handles around it. Double click within the text area box, and a Properties box will appear.
  4. One of the elements in this box is WordWrap. This will say OFF. Highlight it and change to VIRTUAL (use caps)
  5. Close the Properties box
  6. Then turn Design Mode off by reselecting it. You should now be able to type in the text area you selected, and the text will wrap when you get to the end of a line.

Please note: If you cannot fit all supporting information in the boxes provided on this form, please attach further details. (You will know you have run out of room when the scroll toolbar becomes activated. This will make text unviewable when printed.)
