Before You Start - Important Information on Application Forms
- Before you submit an application form please read it carefully to ensure that you have completed all of the details. If you have any queries please contact IT Help Desk on 2-5000 (internal) or 8302-5000 (external), Monday - Friday, 8:30 am - 9:00 pm (ACST). Please clearly print all information requested. Forms with missing and/or illegible information will delay your application.
- Please make sure all required signatures are obtained. An application without all required signatures will considerably delay your access.
- If the application form requests a University Postal Address, please write the UniSA internal mail address in the space provided. If not completed correctly, your notification will be lost.

Create an Account (download form)
This form is for staff members who do not have Continuing, Fixed Term or Casual (CAS) contracts with the University yet require network accounts and/or email access. This includes, but is not limited to, unpaid staff such as adjuncts, temp agency staff, staff paid by invoice, contractors, off shore partners, and PhD students who may require staff access. Staff from external suppliers such as SAIBT, CELUSA and USASA also require this form.
This application should be completed by the university staff member's supervisor/manager.
Delete an Account (download form)
This form is for deleting existing university staff email and/or desktop network access.
Request for Data Centre Hosting
ISTS have established high class data centres at the Mawson Lakes and City West campuses and wish to encourage divisions and units within the University to consider hosting their equipment in one or both of these. Each data centre provides redundant power, air-conditioning and network connectivity along with a sophisticated fire detection and suppression system. The data centres are connected via a redundant, high speed network.
To make use of Data Centre services you must read and agree to conditions in the Data Centre Hosting Policy and complete a Data Centre Hosting Request form and IP Allocation Details form for each piece of equipment that you wish to place in the Data Centre.

Request a Temporary Account for a Visitor (download form)
This form is for UniSA staff to request a temporary network access account
for a short term visitor to the University. The form should be used only for
short term visitors or special events held by the University. Note that
all visitor account requests are subject to approval from the IT Security team.

Application Form must accompany all purchase orders for desktop computers and software.
UniSA Dial up and Remote Access
UniSA Dialup Access (download form)
This application is for UniSA dialup access. Use of this form is restricted to university staff and post-graduate Master's and PhD students only.
Access through external ISP
The university does not have a preferred ISP. Users are advised to compare the offerings of various ISPs and consider price, time and download limits, and service. Note that some UniSA systems ("the Intranet") prevent access from outside the university, and no ISP can provide access to these systems.
Software in General Purpose Computer Pools (download form)
This form is for university teaching staff who wish to have specific software installed, modified or deleted in the ISTS General Purpose Computer Pools. Be aware of the minimum four-week lead time requirements and the need to provide documented evidence of the existence of the required software licences or the software publishers licence conditions (for "free" software).

For information about the UniSAinfo data warehouse please visit the UniSAinfo home page at http://www.unisa.edu.au/unisainfo. Note that staff must maintain the confidentiality of all information available in UniSAinfo and access is subject to the Council Approved Policy on the Use of University Information Technology Facilities.

You should complete this form if your School is to fund extra Internet or Print quota(s) for students as part of their study.
Telephone Services Minor Works Requests (download form)
This form is for staff requiring:
- Re-location of existing telephone points
- Installation of new telephones
- Conversion from analogue to digital PABX service (or vice-versa).
- Installation of ISDN services for data, videoconferencing etc.

You should complete this form if you need to make changes to your telephone directory information.
