Telephone Work Requests can be more complicated than they first seem as
contractors or Network Services may need to install extra network hardware
to complete your request. Please
allow ample time for the completion of Telephone Work Requests. If you have
any enquires that are not answered by the following FAQ please contact the
IT Help Desk.
WORK REQUEST INFORMATION needs to be filled out for every request. If it
is not filled out and signed then there will be delays in processing your
request.
All fields that are marked with an asterix (*) are required fields. The form can not be processed unless they are completed.
SECTION ONE is for new installations only. New installations involve
connecting a network point to the PABX. If you need a new network point
installed the job will take longer and cost more to complete.
- The name of the person who will use the phone or a short description
of what the phone line is for, e.g. Campus Central spare or E1-16 Server
room phone
- The location of the phone point/network point you want to use, that
includes Campus, Building and room number
- The wall point number, if there is no network/phone point then one
will need to be installed. Please tick the Additional wall point
required box. This takes time and will increase the cost of the job. To
help us install the network point in the right location please place a
Post-It note in the location you desire the point to be installed.
In addition you can attach a map in the additional information section
on page two of the Telephone Work Request form.
- The type of phone line you want, either analogue or digital. A fax
or modem line must be an analogue line.
- If new handsets are required please state how many. For

- If voicemail is required please tick the correct box ($100 set up
will be charged to your cost centre).
- The type of access your phone needs. Local is for calls within SA
including calls to mobile phones, STD is for national calls and IDD is
for international calls.
- If an new extension is required on a particular day please fill in
the Required Date. Please note that since each campus is allocated a
single day each week for telephone changes extra charges will apply if
changes are requested on non allocated days. Follow the web link below
for details on which Campus is serviced on each day.

SECTION TWO is for moving extensions to a new location.
- To facilitate the move you need to fill in ALL the fields. If you
need an additional wall point tick the associated box and provide a map
of the location you want it installed.
- The Phone Contact Name needs to be filled in with the person who will be
using the phone in its new location.
- Moving an extension to a new location will leave the Existing Wall
Point number disconnected.
- If an extension move is required on a particular day please fill in
the Required Date. Please note that since each campus is allocated a
single day each week for telephone changes extra charges will apply if
changes are requested on non allocated days. Follow the web link below
