How do I fill out the Telephone Work Request form?

NEC VoIP phones can moved to any active staff network point without making any changes to PBX system. If the move is a long term move and/or you want to ensure that your phone will continue to work in a disaster recovery scenario please ensure you either fill out the TWR move form or complete the online Directory update form.

Telephone Work Requests can be more complicated than they first seem as contractors or Network Services may need to install extra network hardware to complete your request. WORK REQUEST INFORMATION needs to be filled out for every request. If it is not filled out and forwarded to the IT Help Desk from the Cost Centre Manager via email, there will be delays in processing your request.


Telephone Work Request - New Extension. New installations involve connecting a network point to the PABX. Jobs requiring installation of new network points take longer and cost more to complete.

The following numbers correspond with the numbered items on the form

  1. The date of the application
  2. The name of the person requesting the work.
  3. Contact number of the person requesting the work.
  4. Mobile number of the person requesting the work (if applicable).
  5. Cost Centre Code for the work request.
  6. Cost Centre Code for the on-going call charges.
  7. Date the work is required to be completed on.
  8. The campus the telephone extension is on.
  9. New telephone extension contact name.
  10. Building and room number of the new telephone extension
  11. Wall point number for the new telephone extension
  12. Is an additional wall point required? Please note: you will need to allow 10 working days for the installation of new wall points before your TWR can be processed.
  13. ORG2 code for the new telephone extension staff member
  14. Type of connection required (analogue or VoIP) - Justification for a new analogue line needs to be provided if it is not for a fax line.
  15. Is the new telephone extension for a fax line?
  16. Is a new handset required? - For additional information on phone costs, please visit our FAQ:  How much is my Telephone/Network work going to cost me?
  17. Is Voicemail required? - Please provide the user details for the voicemail to be attached to.
  18. What level of call access is required (default is internal dialling only)
  19. Any other comments or diagrams that will assist us in completing your request.

Telephone Work Request - Move Extension is for moving extensions to a new location.

The following numbers correspond with the numbered items on the form.

  1. The date of the application
  2. The name of the person requesting the work.
  3. Contact number of the person requesting the work.
  4. Mobile number of the person requesting the work (if applicable).
  5. Cost Centre Code for the work request.
  6. Date the work is required to be completed on.
  7. The campus the telephone/s is on.
  8. ORG2 code for the staff member
  9. For each move please list the following details
    • Name for the extension
    • Current extension number
    • Moving from room number
    • Moving from wall point number
    • Moving to room number
    • Moving to wall point number
    • Phone type
    • Is a new handset required? - For additional information on phone costs please visit our FAQ:  How much is my Telephone/Network work going to cost me?
    • Is new Voicemail required?
    • Date of the move
    • Any comments relating to this particular extension number.
  10. Any other comments or diagrams that will assist us in completing your request.
