To be entitled to take maternity leave you must provide the University with a medical certificate, no later than ten weeks prior to the expected delivery date, stating that you are pregnant and the expected date of birth.
Simply fill in the HRISOO8 leave form and present to your manager no later than four weeks before the expected date of birth or adoption leave date so that necessary arrangements can be made in relation to your absence.
