How do I fill out a Commonwealth Assistance Form?

The Commonwealth Assistance Form (CAF) is filled out online as a part of the enrolment checklist on myEnrolment. If you have already completed the checklist, you can access it via the myDetails section of myEnrolment and then a link to CAF on the left hand menu.
