People living with serious Mental Health problems have a reduced life expectancy of up to 15 years, with the biggest cause of death being Cardiovascular disease.
This half day workshop will assist health care professionals in identifying and supporting people living with a serious mental health problem in accessing primary health care services. Participants will receive training on how to use the Health Improvement Profile’ to complete a physical health care assessment on their clients and to plan a program of care.
The training programme is interactive and will emphasise the rationale for addressing physical health in people living with serious mental health problems. Participants on completion of the training will have an opportunity to participant in six one hour “community of practice” sessions. The “community of practice” allows participants to discuss and reflect on their experiences of using the skills covered in the training programme.
